Greetings, Over the weekend our team pushed the following patches for the game. Released 6/26 on PC:

• PC: Fixed a problem with Steam achievements resetting and not accumulating progress

Released 6/28 on PC; (PS4 and Xbox One should be live 7/3):

• Xbox One: Fixed a problem with achievements not accumulating progress
• Xbox One: Fixed a problem with Hazmat DLC content not appearing
• PS4: Fixed a problem with Circle and Cross button mappings, for the Japan locale
• UI: Fixed a problem with selecting weapon charms in the Customize screen
• Sunnyvale: Fixed an AI navigation problem, at the car with the car alarm
• Radio Silence: Fixed a texture problem on a car
• Nuclear Facility: Fixed a problem with an electrical trap starting the map in the on state.
